**Ohio no longer requires a proctored final exam. However, a signed affidavit and passing results must be returned before the certificates can be issued. Kentucky & Indiana do not require signed affidavits.

After passing the exam for each line of authority, the original registration form, the original affidavits signed and dated by the student and a copy of the test results must be sent to the Cincinnati Insurance Board within fifteen days of the examination. We will then email you your certificate of course completion. Please allow 5 days for processing & delivery. Only original emailed certificates of completion will be accepted by PSI Services, LLC, the state test administrator.
